25 | On 12/03/2025, Licensing Program Analyst (LPA) Aurelio Mendoza conducted an unannounced case management inspection. LPA met with Director Erin Ortiz, toured the facility, and conducted a census. This visit was consultative in nature and focused on reviewing reporting requirements for unusual incidents.
LPA discussed California Code of Regulations, Title 22, Section 101212(d)(1) and (2), which requires facilities to report specific incidents, such as injuries requiring medical treatment, suspected abuse, or any event that threatens a child’s health or safety, by phone or fax within the next working day, followed by a written report within seven days. The written report must include the child’s information, date and nature of the event, physician findings if applicable, and the outcome.
LPA emphasized the importance of providing full context in incident reports, including who was involved, what occurred, when and where it happened, how it happened, staff-to-child ratios, and actions taken. LPA also reminded the Director of the importance of promptly informing parents and submitting reports to the Department within regulatory timelines.
A printed copy of the regulation was provided and reviewed. Technical assistance was offered to support ongoing compliance.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiencies were cited. An exit interview was conducted with Director Erin Ortiz. A copy of the report, Appeal Rights, and a Notice of Site Visit were provided and discussed.
