25 | On 02/07/2025, Licensing Program Analyst (LPA) Stephanie Vega-Gonzalez conducted an unannounced Case Management Inspection. LPA met with Teacher 3, Jessica Denn who accompanied LPA during a tour of the facility and took a census. LPA counted a total 96 children. LPA observed that a LIC308 for designation of facility responsibility was in place for Teacher 3, Jessica Denn. LPA explained the purposed of the inspection. The purpose of the inspection was due to the Department receiving information regarding an incident that occurred with Child #1 on 01/16/2025. A report to the Department was made on 01/22/2025. It was reported that child was left without supervision for a proximately five (5) minutes in the outdoor play area. LPA Vega-Gonzalez was made aware by Jessica Denn that all staff obtained a training on 02/04/2025 regarding the Supervision of Children in care. LPA obtained copies of training materials.
Due to the incident of a child being left unsupervised, this center is being cited a Type A under the Title 22 CCR 101229 Responsibility for Providing Care and Supervision.
LPA reviewed the Title 22 CCR 101212 Reporting Requirements with Teacher 3, Jessica Denn. Regulations states that, “…a report shall be made to the Department by telephone or fax within the Department’s next working day and during its normal business hours…” This requirement was not met and a Type B deficiency was cited. The incident had occurred on 01/16/2025 and report was made on 01/22/2025.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiencies are being cited: (see next page). Upon receipt of a Type A violation, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. A copy of the Fact Sheet - Child Care Parent Notification Requirements and a copy of LIC 9224 Acknowledgement of Receipt of Licensing Reports was given to Teacher 3, Jessica Denn.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
