32 | · 12/11/2019 – 101221(a) Child’s Records; “A separate, complete and current record for each child is maintained in the child care center.” On 12/11/2019, LPA Gavoutian reviewed eight preschool children’s files, six infant children’s files, and four school age children’s files. Of the files reviewed LPA Gavoutian observed that three did not contain a signed admission’s agreement; 10 did not contain physician’s reports; six did not contain immunization records; 13 did not contain TB test assessment; one child was present who did not have a facility file.
· 12/11/2019 – 101217(a) Personnel Records; “The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee.” On 12/11/2019, LPA Gavoutian reviewed six staff files and observed two files did not contain a health screening; one file did not contain record of immunizations for measles and pertussis and four did not contain record of influenza.
· 12/11/2019 – 1596.8662(b)(3) Availability of information regarding detecting and reporting child abuse and neglect; training for mandated reporter who is licensed day care provider, administrator, or employee of a licensed child day care facility; proof of completion; “On and after January 1, 2018, a person who becomes an administrator or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a) within the first 90 days that he or she is employed at the facility and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.” On 12/11/2019, LPA Gavoutian reviewed six staff files and observed six records did not contain documentation of AB1207 mandated reporter certificates.
· 12/11/2019 – 1596.8595(c)(1) Posting licensing report by child care facility or home; duration of posting; civil penalty for failure to comply; reports to be provided to parents or guardian of each child receiving services; “A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care as set forth in paragraph (1) of subdivision (a) of Section 1596.893b.” On 12/11/2019, LPA Gavoutian reviewed eight preschool children’s files, six infant children’s files, and four school age children’s files. Of the files reviewed LPA Gavoutian observed that eight did not contain signed LIC 9224 – Acknowledgement of Receipt of Licensing Reports for a Type A deficiency that was cited during a case management inspection conducted on 06/05/2019.
