25 | On 08/21/2023, Licensing Program Analyst (LPA) Ka Vang conducted an unannounced case management inspection. LPA met with Site Supervisor Dominique Aldez-Ferguson. A tour of the inside facility was conducted, and a census was taken. LPA explained to Dominique the purpose today's inspection was to discuss an unusual incident that occurred on 08/16/2023, in which child #1 received a head injury in the Dino Room (Infant Room). This incident was reported to the Fresno Community Care Licensing Office on 08/17/2023 by Site Supervisor Dominique.
Dominique stated that on 08/16/2023, there were nine (9) infants and three (3) qualified staffs presented during free time play at 10:00 AM. Staff #2 was with four infants on one side of the room and Staff #1 and Staff #3 were with four (4) other infants on the other side of the room. Dominique stated that around 10:15 AM, Staff #1 brought infant #1 to her office and stated that infant felt backward and hit her head which was bleeding. Staff #2 applied first aide to infant by putting pressure on infant’s wound. Dominique immediately contacted parent via phone at 10:15 AM. At 11:15 AM (based on record sign-in and sign-out), infant was picked up by parents.
Interviewed with conducted with Staff #2 and staff #2 stated that at 10:00 AM, her and four (4) was in the classroom and they were engaged in active play on the floor near the infant rectangle table (w/ 2 chairs) and as all the four infants and her were actively play with the balls, infant #1 got up and lost her balance and instantly fell backward and her head hit the rectangle table. As infant #1 was crying, staff #2 immediately picked up infant and providing comfort while checking on infant and staff #2 observed that there was blood on the back of infant #1’s head. Staff #2 immediately applied pressure to infant’s open wound, at the same time, staff #1 was called to observed. Staff #1 then took infant #1 to site supervisor. Staff #1 stated that ice packet was not applied due to infant’s open wound. During the incident, staffs and site supervisor observed child was still awake; therefore, emergency personnel was not contacted.
