- 6/28/2021 - 101416.3(b) Infant Care Aide Qualifications and Duties; "An infant care aide shall work under the direct supervision of the director, the assistant director or a fully qualified teacher, except as provided for in Section 101416.5(d)(1)." On 6/28/2021, LPAs Luisa Gavoutian and Stefanie Galvan entered the infant classroom and observed that Staff 1 was in the infant classroom alone. LPAs reviewed Staff 1's records and found that Staff 1 has not completed the early childhood education or child development units required to provide care and supervision to infants alone.
- 6/28/2021 - 101223(a)(2) Personal Rights; "To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs." On 6/28/2021 LPAs Luisa Gavoutian and Stefanie Galvan observed in the infant classroom, Child 1 in a crib, which had been rolled out into the classroom area. Child 1 was awake and Staff 2 was interacting with Child 1 with a rattle. Staff 2 did not meet accommodations to the needs of Child 2, but instead handed Child 2 to Staff 2.
- 6/28/2021 - 101229.1(b) Sign In and Sign Out; "The person who brings the child to, and removes the child from the center, shall sign the child in/out." On 6/28/2021 LPAs Luisa Gavoutian and Stefanie Galvan attempted to review facility's sign-in/sign-out records and found only Child 3 and Child 4 has sign-in/sign-out records completed. Licensee stated that due to pandemic she was asking parents to complete sign-in/outs at the end of the month only.
- 12/12/2019 - 101439(d)(2) Infant Care Center Fixtures, Furniture, Equipment and Supplies; "A baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Section 1596.846." On 12/12/2019 LPA Luisa Gavoutian observed a baby rocker being used.
- 12/12/2019 - 101238.2(d)(2) Outdoor Activity Space; "The surface of the outdoor activity space shall be maintained. Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard." On 12/12/2019 LPA Luisa Gavoutian observed an opening in the fence of the infant outdoor play area that leads into a larger field that contains various hazardous items such as two large tree trunks of trees that were recently removed, and various large items. LPA also observed this are also leads directly into the street. (Continued on LIC 809-C)
