On 12/12/2024 Licensing Program Analyst (LPA) Stephanie Vega-Gonzalez and Aurelio Mendoza, conducted an unannounced Case Management Inspection Initiated by the licensee for the preschool license. LPAs met with FUSD-Thomas Principal Matthew Phanco and toured the facility indoors and outdoors and a census was taken.

The purpose of today’s inspection was to review the shared restroom waiver request that was submitted to the Department on October 25, 2024, and to inspect the restrooms and classrooms. LPAs reviewed information with FUSD-Thomas Principal Matthew Phanco. LPAs took photos of restrooms and classrooms. All gathered information regarding the waiver is currently pending review from the Fresno Community Care Licensing Office. FUSD-Thomas Principal Matthew Phanco was informed by LPAs that communication would follow once the final decision had been determined.

Exit interview conducted and report was reviewed with FUSD-Thomas Principal Matthew Phanco. Appeal Rights were provided.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
