25 | On December 29, 2021, Licensing Program Analyst, Brannon, met with Lead Teacher and Assistant Director, Amanda Tuller. During today's inspection, LPA toured facility, took a census, reviewed staff and child's files, interviewed staff and received documentation.
LPA received an unusual incident report for an incident that took place on November 8, 2021. Director, Maryellen Price, called the Department, informing that a parent made an allegation against staff 1 (S1). The parent was told by child #1 (C1) that S1 pushed C1 head into the cot in an aggressive manner during nap time. Licensee conducted several meetings with parent. One meeting was held on November 9, 2021 with Assistant Director, S1 and parent of C1. The second meeting was held on November 17, 2021 with Director M Price, Assistant Director/Lead Teacher Amanda Tuller and parent of C1. After the first meeting, it was decided to have S1 out of the classroom for the rest of the week. S1 returned to the classroom on November 15, 2021.
During today's inspection, LPA interviewed Director, Assistant Director and staff #2 (S2). LPA toured the room where the alleged incident took place. S1 was on vacation and C1 no longer attends facility. Per interviews, S1 was not observed to have pushed C1 head into the cot, nor was C1 heard or seen crying during nap time. S2 was sitting across from S1 and C1. S2 had visual observation of C1 and S1. S2 did not see S1 push C1 head into the cot, nor heard C1 crying or making any statement about S1 pushing head into cot. Based upon interview findings and documentation received during today's inspection, licensee took appropriate steps to ensure the safety of children in care.
Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited. Licensee was provided a copy of their appeal rights.

This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
