On 12/21/2023, Licensing Program Analyst (LPA) Stephanie Vega-Gonzalez conducted a Case Management to review past deficiencies dated from 9/6/2023 and 11/9/2023. The purpose of the inspection was to review the Plan of Correction for the deficiencies cited on 9/6/2023 and 11/9/2023. Today, LPA met with Director, Sandra Graham. LPA toured the facility, and a census was taken of 8 preschool children and 2 school age children.

During the initial tour LPA observed that Staff 3 and Staff 5 were in the preschool classroom with a census of 10 children. LPA observed that 2 out of the 10 children were school age children. LPA discussed with Director, Sandra Graham that preschool children and school age children can not be commingling, and that school age children need to be in the school age classroom with a qualified staff who will be supervising and caring for children. Director, Sandra Graham stated that she understood. LPA observed as Director moved school age children from preschool classroom and asked children to sit and play with toys in the common dining area. A deficiency was cited in the school age license.

On today’s date LPA’s purpose was to review deficiencies with Director, Sandra Graham. LPA was able to clear two deficiencies during today's inspection and provided copies of letters to Director, Sandra Graham. LPA printed deficiencies from 9/6/2023 and 11/9/2023 and reviewed them with Director, Sandra Graham. LPA provided a new due date for all deficiencies, 12/29/2023. LPA reviewed reporting requirements with Director, Sandra Graham.

During record review it was observed that Director, Sandra Graham did not have a complete file. LPA reviewed with Director, Sandra Graham all the missing documents which also consist of her Mandated Reporter, immunization records, and center director packet. LPA had requested required director documentation since 10/30/2023. LPA reviewed with Director, Sandra Graham the required documents and provided a due date of 1/22/2024. Director, Sandra Graham understands that she will stay in communication with LPA and the Department during this process.

LPA also observed that Staff 1, Staff 2, and Staff 4’s files were not present at facility. LPA had requested proof of files for Staff 1, Staff 2, and Staff 4 since 9/6/2023. The Department had not received proof of these corrections. Director, Sandra Graham stated that she will work on deficiencies and submit proof of corrections to the Department.

Exit interview conducted and report was reviewed with Director, Sandra Graham. Appeal rights were provided.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiencies are being cited: (see next page). There were one civil penalties assessed on today's date for repeated violations.

This report shall be made available to the public upon request.

LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
