25 | On 03/22/2024 Licensing Program Analyst (LPA) Stephanie Vega-Gonzalez conducted an unannounced inspection and met with Staff 1, who has Designation of Responsibilities at 9:45 a.m. Also present was Staff 2 and Staff 3. LPA was informed that Director, Sandra Graham was going to be in shortly, due to running a quick errand. LPA explained the reason for the unannounced inspection, showed badge, provided copies of business card, and conducted a census count and an inspection inside the center and the play yard. LPA observed three preschool children, yet no school age children were present during the unannounced inspection. Director, Sandra Graham arrived at 10:20 a.m..
On 3/21/2024 the department was notified of an incident that had occurred with Child 1. The department was notified that Child 1 was not picked up from their designated place of pick up. On today's date, LPA reviewed facility records. LPA reviewed Plan of Operations for Transportation. LPA obtained copies of the transportation contract. Based upon observations, and information gathered through interviews and facility records a Type B citation was issued on today's date under 101173 Plan of Operation (d) The child care center shall operate in accordance with the terms specified in the plan of operation. LPA gained evidence that center did not follow the center's plan of operation/ transportation contract/ policy.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page).
This report shall be made available to the public upon request.
LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
