13 | On 10/07/2022, Licensing Program Analysts (LPAs) Ka Vang and Cynthia Brannon arrived at the facility to conducted an unannounced follow-up inspection to conclude the complaint investigation that was initiated on 07/27/2022. LPAs met with Ezula Avila and discussed the findings of the investigation. A tour of the facility was conducted both inside and outside. Census was taken.
During the course of the investigation, LPAs interviewed facility staff, day care children and other who had knowledge of the allegation and it was determinate that day care children are allowed to use the restroom without staff supervision.
Based on the interview and information attained during the investigation, there is a preponderance evidence that the facility left the day care child without supervision. Based on the above information, the allegation is found to be SUBSTANTIATED. A type “A” violation will be cited.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, the following deficiency is being cited on the attached LIC 9099D, see next page.
