32 | This requirement was not met as evidenced by interviewed and observation as Licensee failed to report that the facility was closed on 07/27/2022 due to the children’s toilets were clogged and caused an odor in the facility and the facility running water was shut off because of the toilets clogged. This poses a potential health, safety, or personal rights risk to persons in care.
On 10/18/2022, an Informal Meeting was completed at the Fresno Regional Office. During the meeting, Licensee, Licensing Program Manager (LPM) and Licensing Program Analyst (LPA) agreed that Licensee is to submit the Unusual Incident Report (UIR) occurred on 07/27/2022 by 11/07/2022. On 10/19/2022, Licensee submitted the UIR for 07/27/2022. The deficiency was clear on 11/9/2022 but Licensee submitted the POC timely by 07/27/2022.
During today’s inspection, LPA observed the facility have 2 restrooms located in the preschool area classroom. One of the restrooms is for the preschool boys, school age boys and school age girls. The second bathroom is for the preschool girls. LPA observed that there was one toilet located in the first restroom that had the stall installed to allow privacy for the school age boys and girls. Licensee is to ensure that there are protocols that when the preschool boys in the bathroom then the school age boys or school age girls are not allow in or use the bathroom to prevent co-mingling.
LPA reviewed documentation and observed the facility inside and outside and licensee met the POC. During this inspection, LPA did not observe any procedures and no regulations were violated.
