25 | On 07/29/2025, Licensing Program Analyst (LPA) Valerie Mireles conducted an unannounced case management inspection. LPA met with Director Barbara Daniel. The purpose of today's inspection was to gather more information regarding an unusual incident reported to Community Care Licensing on 07/15/2025, involving a child’s personal rights being violated.
During today’s inspection, LPA obtained documentation pertinent to the incident, interviewed Director Barbara Daniel and staff. Director and staff interviewed explained the facility policy regarding handling challenging behaviors and the way redirection techniques are applied at the facility. Additionally, Director and staff interviewed explained that they practice a hands-off approach towards responding to challenging behaviors.
Information obtained through staff interviews revealed that, on 07/14/2025, Staff #3 held Child #1 by the arm and in an aggressive tone stated, “You aren't listening.” Staff #3 then grabbed Child #1 by the chin, directing the child’s face toward Staff #3. Facility took appropriate measures to intervene, address the incident and met reporting requirements.
LPA determined Staff #3 mishandled and used an aggressive tone toward a child in care. This poses a potential risk to the health, safety, or personal rights of children in care.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page, 809 D). Exit interview was conducted with Director, Barbara Daniel and a copy of appeal rights was provided.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
