25 | On 11/08/23, Licensing Program Analyst Denisia Jimenez conducted an unannounced case management inspection. LPA met with Program Manager, Barbara Daniel. The purpose of the inspection was regarding an incident report that was submitted to Community Care Licensing regarding an incident that occurred at the facility on 10/31/23.
Barbara stated that on the date of the incident, Child #1 was outside and went back inside the classroom to get their coat. Staff #1 was holding the door for the children to go outside. Staff #2 was inside the classroom, carrying the water canister and passed by Staff #1 and walked outside. Staff #1 was still standing there holding the door when another child approached Staff #1, started talking about something and Staff #1 let go of the door. Before all staff and children went outside, one of the staff had contacted Staff #3 to unlock the doors, it’s an everyday safety practice. Staff #3 went to the door to unlock it when Staff #3 saw Child #1 standing by the door. Staff #3 asked child #1 what child #1 was doing in the classroom and child #1 stated he was looking for their coat. Staff #3 helped child look for their coat and once found, Staff #3 took the child outside to play with the rest of the children. Barbara stated Child #1 parent was contacted and explained to the parent the incident. The parent understood. Through interviews conducted, it was confirmed that Child #1 was left unattended for 1-2 minutes. LPA will issue a Type B citation. LPA spoke to Barbara and reminded them to have visual supervision of children at all times whether they are outside or inside the classroom. Barbara indicated that she provided supervision training resources and reminded all staff of the importance of child supervision, keeping count of children at all times, and communicating with other teachers present when taking children indoors.

cited (see 9099-D)
Exit interview conducted report and appeal rights was reviewed with the facility Program Director Barbara Daniel. This report is to be made available to the public upon request. LIC 9213 Notice of Site Visit to be posted for 30 days.
