25 | On 03/17/2026, Licensing Program Analysts (LPAs) Jeovanna Yanez and Sha Fee Vang conducted an unannounced Case Management - Incident inspection and was met by Site Manager, Kimberly Nunn. LPAs toured the facility and a census was taken. The purpose of the inspection was to obtain additional information about an incident that was reported to Community Care Licensing (CCL) - Fresno Regional Office on 03/10/2026 regarding children's personal rights.
During today’s inspection, LPAs interviewed staff, made observations at the facility, and reviewed records pertinent to the incident. Through interviews conducted, On 03/09/2026 around 4:30 PM it was reported that Staff #1 was near Child #1 who was crying and started throwing Magna-Tiles toward Child #2, when Staff #1 attempted to calm down Child #1 and prevent them from hurting another child. Staff #1 was about to place their hand on Child #1's back, when Child #1 began to walk away and Staff #1's hand was guiding child away from the area. The facility follows a "Hands-Off Approach" for supporting children with challenging behaviors, and Child #1's parent was notified of this incident on date of occurrence.
Based on the information obtained during the inspection, LPA Yanez determined that no child’s personal rights were violated. Furthermore, this was an isolated incident and staff took appropriate measures to address the incident, following appropriate policies, regulations, and reporting requirements.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiency is being cited: A copy of appeal rights was provided. An exit interview was conducted with Kimberly Nunn.
This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
