25 | On 09/12/2023, Licensing Program Analyst (LPA) Martha De Haro conducted an unannounced case management inspection. LPA toured the facility, and a census was taken. LPA met with Director, Lindsi Pipkin. The purpose of today's inspection was to discuss an unusual incident, which occurred on 08/15/23 in which child #1 stated that a teacher had pulled his ear during class. Child #1 had only been at the center for two days.
During the case management inspection, LPA interviewed the Director and preschool teacher, reviewed facility files, reviewed protocols for reporting obligations, and received and gathered additional documentation.
After conducting the case management inspection and interviewing both the Director and the preschool teacher, it was found that child #1 has a history of behavioral issues at the center and at a previous center. Per Director, during child #1’s enrollment, child’s mother had informed staff that child had previously made a similar accusation at a previous center. The preschool teacher at this center denied ever pulling child #1’s ear. Per documentation, preschool teacher does not have a history of inappropriate conduct at the facility. Due to several behavioral issues which resulted in child #1 being suspended from the center, Director held a meeting with the child’s mother on 09/12/23 in order to come up with a Behavioral Guidance Plan to address the child’s behavioral issues. Staff took appropriate measures to investigate and address the incident and additional incidents involving child #1, following appropriate policies, regulations, and reporting requirements.
Per Chapter 1, Division 12, Title 22 of the California Code of Regulations, no deficiencies were cited during today’s inspection.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with Director, Lindsi Pipkin and a copy of the report and appeal rights were given and discussed.
