25 | On 11/3/23, Licensing Program Analyst (LPA) Priscilla Zamudio conducted an unannounced case management inspection. LPA met with Cinthia Espino, Teacher (Site Supervisor) to discuss an incident which occurred on 10/18/23 and a census was also taken. LPA interviewed staff and observed where the incident occurred.
On 10/18/23, at approximately 10:00 am, child #1 went to the nurse’s office with an Aide, due to not feeling well. The Aide left the child with the nurse and informed the nurse that the class would be on the playground if they needed to be contacted. Around 10:35 AM, the class returned to the classroom. As they were settling on the carpet, child #1 was observed alone, outside the classroom gate, and was brought inside by staff. When the child was picked up, the teacher spoke to the parent and explained what happened. Immediately after all the children went home, the teacher spoke to the nurses to seek an explanation and let them know preschool students are never to walk alone on campus. The teacher also met with administration to discuss the incident and made a plan to prevent this from happening again.
Based on information gathered and observations made by LPA, there were no obvious concerns or hazards found. Staff are implementing adequate care and supervision at the facility.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.

This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
