| Director Sal Fonseca was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
This facility does not provide infant care at this time.
The following items must be completed prior to issuing a license by 08/15/2023:
1. Replace foam and artificial grass by monkey bars and in front of two side-by-side slides.
2. Remove spider webs from plants by outside play area.
3. Address condensation from air conditioner pipe dripping from roof onto play area.
4. Letter of Attestation for the drinking water stating that facility is utilizing water delivery, along with copy of water delivery contract.
5. Picture of igloo with disposable cup dispenser and disposable cups.
6. Letter of Employment Verification for Liliana Rubio
7. LIC 500 (Employee Roster)
8. LIC 610 (Emergency Disaster Plan) with two relocation letters
9. Orientation Certification for Component 2 (Operations and Record Keeping) for Liliana Rubio
10. Provide the specifications for the artificial grass and rubber foam cushioning.
LPA and LPM reviewed with Director Sal Fonseca, the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
Upon receipt and review of documentation, a recommendation to license a preschool center with a capacity of 23 preschool children will be made.
