On 09/10/2026, Licensing Program Analyst (LPA), Stephanie Vega-Gonzalez conducted an announced case management inspection that was initiated by licensee, Licensee is Fresno Unified School District, for a change of components and classrooms. This facility is currently licensed, yet does not have any children enrolled. Upon arrival, LPA met with Facility Representative/Director, Charlotte Miranda.

The applicant is now requesting to be licensed for 9 infants ages 0 months to 24 months of age in the infant classroom; 12 toddlers ages 18 months through 36 months of age in the toddler classroom; 40 preschool children ages 3 years through 5 years of age in the preschool classroom 1 (PS 1) and preschool classroom 2 (PS 2). Hours of operation will be Monday through Friday from 7:45am through 4:45pm for all three components. This facility runs year round.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed the Director that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, the director stated they will be in the schools nurses office located in the main front office. A fully equipped first aid kit is located in infant, toddler, and preschool classrooms, as well located in the front office. There is an operational carbon monoxide detector on site located in all the classrooms. The infant and toddler classrooms are found in one classroom with a partition wall dividing both indoor areas for each component. This building is labeled as “(E) Building & Infant/Toddler” on the map. The Preschool classrooms are labeled as “(E) Building 1 Classrooms and workrooms in room number 3 and 4” on the map. All required licensing documents were posted in the Infant and Toddler Classroom entrance and in each of the Preschool Classroom 1 and Preschool Classroom 2. Children will be signed in and out in the Toddler Classroom by the entrance way. The infant and toddlers will be singed in an out at the entrance hallway of the building.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The preschool children will be signed in and out at the entrance hallway/door of each classroom. The parents will need to come from the front of the property/office and walk to each respected classroom to drop off or pick up their child.

LPA continued to tour the facility and measured all indoor and outdoor activity space. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via filter water pitchers and disposable cups. LPA observed all hazardous items to be inaccessible to children. Facility Representative stated that there are no bodies of water, any water features, or weapons on the property. Fire clearance was granted on 08/26/2026 by the Fresno City Fire Department.
Infant Indoor Activity Space:
The total indoor activity space measures approximately 328 square feet, which will accommodate Applicant’s request for 9 infants.
Toddler Indoor Activity Space:
The total indoor activity space measures approximately 554 square feet, which will accommodate Applicant’s request for 12 toddlers.
Preschool Indoor Activity Space:
The total indoor activity space measures approximately 1344 square feet, which will not accommodate Applicant’s request for 40 preschool children. Per the measurements, the square footage can only accommodate 38 children. LPA reviewed findings with Facility Representative an updated LIC200A was provided to LPA during the inspection.

LPA observed a total of 14 sinks and 8 toilets available for children’s use. LPA observed that the infant component has access to 1 sink and has potty chair, which is sufficient to accommodate the requested capacity of 9 infants. LPA observed that the toddler component has 4 toilets and 3 sinks which is sufficient to accommodate the requested capacity of 12 toddlers. LPA observed that Preschool Classroom 1 has 5 sinks and 2 toilets, Preschool Classroom 2 has 5 sinks and 2 toilets which is sufficient to accommodate the requested capacity of 38 preschool children.

In the infant room there are low feeding chairs and feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. LPA observed that the changing table is located inside the Toddler Restroom 2. Director stated that changing table will be rolled closer to the sink when in use to ensure it is within arm’s reach. The changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high. Diapering sink is not used for meal preparation or dishwashing. There are 9 cribs that meet regulatory requirements. Director stated that they will have 9 sleeping crib babies enrolled and have cots available. Crib area is separated from activity space via a wooden and plexi glass wall. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs are kept in infant sleeping area and toddler classroom. Needs and Services Plans are kept in the infant and toddler classroom.
The facility currently has a fully fenced playground area. Fencing is made of both wrought iron and chain link fence. LPA observed that the preschool playground is made of wrought iron fence, and the infant and toddler outdoor fence is made of chain link, and is at least four feet high.

Infant Outdoor Activity Space:
The total outdoor activity space measures approximately 2408 square feet, which will accommodate Applicant’s request for 9 infants.
Toddler Outdoor Activity Space:
The toddler component does not have their own separate outdoor activity space. The Licensee has submitted a waiver request for the infant and toddler components to share the outdoor activity space at separate times. The final determination remains subject to Department review.
Preschool Outdoor Activity Space:
The total outdoor activity space measures approximately 12,115 square feet, which will accommodate Applicant’s request for 40 preschool children.

Yet, per the indoor square footage taken on today’s date, the indoor can only accommodate 38 preschool children. Applicant updated request to 38. The outdoor activity space will accommodate the updated request of 38 preschool children.

Shade is provided via shaded canopies found in both the infant/toddler and preschool yards. There are sufficient outdoor age-appropriate toys and play equipment available on the playground.

There is a climbing structure on the preschool playground for children ages 2 years through 5 years of age which is properly anchored. There is a manufacturing sticker on the climbing structure. There is adequate cushioning in fall zones of climber provided by pour in rubber in the preschool yard. There is a small infant climbing structure in the infant/toddler yard that is for ages 6 months to 23 months. Applicant stated that the facility will ensure that only children within that age group will be allowed on the climbing structure. There is adequate cushioning in fall zones of climber provided by pour in rubber in the infant/toddler yard. Drinking water is available via water pitchers and disposable cups for the infants and toddlers and the preschool component will have a water igloo and disposable cups. LPA observed all hazardous items on the playgrounds to be inaccessible to children. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.
Facility will provide breakfast, lunch, and snacks. The infant/toddler classroom has a kitchenette area that is separate from the classroom that also serves as the laundry room. The infant and toddler room has a dishwasher, a dyer and washer, sink, and refrigerator. LPA was informed that facility will provide baby foods, baby formula, and meals for the toddler components for breakfast, lunch, and snacks. LPA was informed that each child will be on their own service and needs plan when it comes to their meals. LPA was informed that breast milk will be provided by parents and stored appropriately if needed. All components meals will be brought in via delivery. LPA observed that there was a refrigerator in each preschool classroom. The kitchenette area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. Hazardous items in kitchen are inaccessible to children via a door in the infant and toddler classroom. LPA did not observe any hazardous items in the preschool classrooms.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPA reviewed with Director the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.
LPA discussed the safe sleep regulations with the director and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communication.

No corrections are needed. A license for 9 infants, 12 toddlers, 38 preschoolers will be reviewed. Final determination remains subject to Licensing Program Manager review.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.
Exit interview conducted and report was reviewed with Director Charlotte Miranda.
A Notice of Site visit was given and must remain posted for 30 days.
