INVESTIGATION FINDINGS:

On 11/07/2024, Licensing Program Analyst (LPA) Valentin Hernandez conducted an unannounced complaint inspection to provide findings for the above allegation. LPA met with Licensee, Annette Lara. LPA reviewed the allegation with licensee. LPA observed 5 children in care. During today’s visit, LPA observed the daycare and delivered the allegation finding.

During the course of this complaint investigation, LPA Valentin Hernandez conducted facility inspections, interviewed the licensee, licensee’s assistants, complainant, children, and parents, and reviewed documentation and files. Due to conflicting information, the compliant finding is unsubstantiated. Although the allegation may be valid, there is insufficient evidence to determine whether the alleged violation occurred.

Per California Code of Regulations, Title 22, Division 12, Chapter 3, no deficiency is cited during today’s visit. Exit interview conducted with the licensee, Annette Lara. A Notice of Site Visit was posted on parent board in presence of LPA Valentin Hernandez.

A COPY OF THIS REPORT IS TO REMAIN IN THE FACILITY FOR PUBLIC REVIEW.
