On 06/09/2026, Licensing Program Analyst (LPA) Aurelio Mendoza met with Applicant Silvia Bahena Ocampo to conduct a capacity increase inspection from a Small Family Child Care Home to a Large Family Child Care Home. The applicant, her husband, and their three minor children reside in the home. Background clearances were discussed, and the LIS 531 was signed, indicating that all adults currently residing in the home and/or providing care and supervision to children have obtained criminal record clearances. Fire clearance was granted on 05/05/2026.

The facility was inspected inside and outside, as shown on the facility sketch, and the following was observed and discussed:

• Fire clearance was received on 05/18/2026. The fire pull alarm is located in the kitchen on the left-hand side wall, near the stove.

• This is a single story home. Children will have access to the living room, dining room, kitchen, family room, bathroom, and backyard. Off-limits rooms and closets are made inaccessible through the use of child safety gates and doorknob spinners.

• There are no fireplaces or open-faced heaters in the home. The home is equipped with central heating and cooling for safety and comfort.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA observed that accessible areas contain children’s books, block activities, child-size furniture, safe toys, and educational activity cards displayed on the walls. Older children will nap in the family room, and the only infant in care at this time will nap in the living room in their own play yard. Applicant understands that children must be supervised at all times. LPA provided the Individual Sleeping Plan and the Safe Sleep handout.

The facility has a 2A10BC fire extinguisher mounted in the kitchen on the wall to the right of the stove. The home contains a functioning smoke alarm, carbon monoxide alarm, and a complete first aid kit.

The applicant’s Pediatric CPR and First Aid certification is valid through 10/12/2027 and includes EMSA-approved stickers. The Preventive Health and Safety with Nutrition and Prevention of Lead Exposure certification was completed on 11/21/2019. The Mandated Reporter Training is valid through 01/05/2028.

Knives and medications are stored in a kitchen drawer to the right of the sink, secured with child safety latches. Cleaning compounds are stored in the garage and made inaccessible by a door lock spinner.

Applicant was advised that fire drills must be conducted at least once every six months and documented with the date and time. A fire drill log was provided as an example. Applicant was also advised that at least one staff member with current pediatric first aid and CPR certification must be on site at all times when children are present.

There are no bodies of water on the premises. Applicant stated there are no pets in the home or on the property. Applicant also stated that no poisons are stored in the home.

Applicant was reminded that all advertising for the child care program, including business cards, flyers, posters, and signs, must include the facility license number, as required by Title 22, Regulation 102359(a), “Advertisements and License Number.” Applicant was advised that smoking is prohibited on the premises of a family child care home, as specified in Health and Safety Code Section 1596.795(a). Applicant stated the home is smoke free.

Applicant stated that she transports day care children. Applicant understands that proper car seats and child restraints must be used for all children when being transported.

The fenced backyard includes children’s play structures and foot powered vehicles. There are no swings. LPA discussed pending safe sleep regulations, and Safe Sleep Regulation Concepts were provided.
Applicant was advised that it is her responsibility to read, understand, and maintain compliance with all applicable Title 22 Regulations. Title 22 regulations can be found at www.ccld.ca.gov. SB 792 immunization records are on file.

Applicant was reminded that the Fresno Community Care Licensing office has inspection authority and may inspect all rooms in the home, the garage, and any separate structures on the premises.

Incidental Medical Services (IMS) were discussed. IMS are not currently being provided. Applicant was reminded that if IMS are provided at any time, a Plan for Providing IMS must be submitted to the Department. The following ADA information was provided: U.S. Department of Justice (USDOJ) toll free ADA Information Line at (800) 514 0301 (voice) / (800) 514 0383 (TTY).

Required postings and a forms packet, including the Parent Notification Requirements and LIC 9224, were provided and reviewed in detail. Applicant was advised that additional forms and updates may be accessed on the Community Care Licensing Division website at www.ccld.ca.gov. Applicant is reminded that it is her responsibility to review regulations periodically. Applicant stated she will operate the child care Monday through Friday from 7:00 AM to 4:00 PM, or as arranged. No overnight care will be provided.

LPA and applicant discussed the Community Care Licensing website, including new PINs (Provider Information Notices) and provider resources such as Quarterly Updates addressing new legislation and regulations. Applicant was encouraged to visit http://www.cdss.ca.gov/, select “Information and Resources,” then “Community Care Licensing,” then “Quarterly Updates,” and register for PINs under the “Child Care Advocates Program.”

Pending final review of the application file, licensure as a Large Family Child Care Home with a capacity of 14 children under 18 years of age will be recommended effective 06/10/2026.
