· Preventative Health and Safety Course with Prevention of Lead exposure certification is scheduled to be completed on September 28-29, 2023.
· Applicant completed the Mandated Reporter Training on April 28, 2023.
· SB 792 immunizations verified and on file.
· Applicant rents/leases the home, proof of landlord notification is required. LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
· In the daycare room, LPA observed wooden cubbies for storage, children size table and chairs, one high chair, a diaper changing table, learning wall art, and safe games, toys, and books for the children. In the living room, LPA observed comfortable furnishings, and a flat screen television mounted on the wall. In bedroom #1, LPA observed one crib, two play yards, child size furniture, and safe toys and books. Children will nap on cots in the daycare room, Infants will nap in crib or play yard in bedroom #1. Applicant understands she is to supervise children at all times.
· Fenced backyard has cement and sodded area for the children. There is a patio porch for shade. LPA observed two Little Tikes plastic slides, a metal swing set that is anchored to the ground, a water table, three Little Tike cars, and other safe toys for the children.
· Facility has 2A10BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives are stored in bottom kitchen drawer made inaccessible via safety latch. Medications are stored in the off-limits master bedroom. Cleaning compounds are stored in a bottom kitchen cabinet made inaccessible via safety latch.
· There are no bodies of water in the home or premises.
· There is one dog and one cat that are in the home. Applicant is advised it is her responsibility to ensure the safety of children in care at all times from the pets.
· Applicant states there are no firearms or ammunition in the home or premises.
