· Applicant completed the Mandated Reporter Training on February 8, 2024.
· SB 792 immunizations verified and on file.
· Applicant provided proof of control of property.
· In the play room, LPA observed children size furniture, a flat screen television mounted on the wall, and safe games, toys, and books for the children. In the living room, LPA observed children size table and chairs, comfortable furnishings, and a flat screen television mounted on the wall. Children will nap on cots in the living room, infants will on play yards in the play room. Applicant understands she is to supervise children at all times.
· Fenced backyard has cement, sodded area, and artificial turf for the children. There is also an area with gravel. LPA observed a patio porch for shade, patio furniture, and various safe toys for the children. There is a two-story wooden play structure with a 4 foot wooden ladder on one side the structure that leads to the second level. The second level has a small observation deck with access to two green plastic slides. There are three swings attached to the play structure. This play structure is anchored to the ground.
· Facility has 3A40BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives are stored on top of the refrigerator. Medications are stored in top kitchen cabinets. Cleaning compounds are stored in a bottom kitchen cabinet made inaccessible via safety latch.
· There are no bodies of water in the home or premises.
· There is a dog that is kept in off-limit areas, and eight (8) fish in a ten gallon fish tank located by the entryway. Applicant is advised it is her responsibility to ensure the safety of children in care at all times from the pets.
· Firearms and ammunition are locked and stored separately, in accordance with Title 22 regulations
· Advised applicant fire drills are to be conducted once every 6 months and must be documented with date and time. A fire drill log was provided as an example.
