· Licensee's Pediatric CPR and First Aid certification was completed through Pediatric Plus with Emergency Medical Services Authority stickers (EMSA) and expires on November 13, 2026.
· Preventative Health and Safety Course with Prevention of Lead exposure certification is scheduled to be completed February 22, 2025.
· Licensee completed the Mandated Reporter Training for Child Care Providers on September 6, 2023.
· SB 792 immunizations verified and on file.
· Licensee rents/leases the home, proof of landlord notification is required. LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. Applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
· In the living room, LPA observed comfortable furnishings, a flat screen television mounted on the wall, children size table, chair, and furniture, a diaper changing table, learning wall art, a small alphabet rug, and various games, toys, and books for the children. Children will nap on mats, infants will nap on play yards in the living room. Licensee understands she is to supervise children at all times.
· Fenced backyard has cement and sod for the children. There is a patio porch for shade. LPA observed patio furniture, a small plastic climbing dome, and safe toys.
· Facility has 2A10BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives are stored on top of the kitchen counter. Medications are stored in the off-limits master bedroom. Cleaning compounds are stored in a bottom kitchen cabinet made inaccessible via child safety latch.
· There are no bodies of water in the home or premises.
· Licensee states there are no pets in the home or on the premises.
· Licensee states there are no firearms or ammunition in the home or premises.
