25 | On 07/25/2024, Licensing Program Analyst (LPA) Jose Penate conducted a Case Management inspection for an incident which occurred on 07/05/24. LPA met with Licensee, Joanna Cortez. LPA toured facility inside and outside and took census.
Licensee, Joanna Cortez stated that child #1 slipped and fell from the stairs playing on the playground structure located at the Park. Licensee stated that she had 4 children in care on the day that incident occured. She stated that she had visual supervision on all children and that child #1 accidentally stumbled over his feet and fell from the stairs.
LPA inspected the playground structure and observed that the structure is safe and did not have any blind spots for lack of supervision. LPA confirmed that the playground has soft cushion floor for all high climbing areas to absorb falls. The playground does not have any signage that advises ages of children that could utilize the playground structure, but LPA advised licensee to always make best judgment if structures or play equipment seem dangerous to avoid any usage. LPA advised licensee to always follow labeling placed on equipment and/or follow manufacture directions.
LPA observed the playground area for any obstructions that would not allow proper supervision by licensee or staff. LPA observed playground to not have blind areas so that visual supervision is maintained at all times.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency cited.
