An unannounced Complaint visit was conducted on the date & times indicated above by Licensing Program Analyst (LPA) K. McClurg. LPA was met at the front door by Care Giver 1 (CG1). LPA introduced self, stated purpose of visit, provided business card, & was allowed entry. CG1 contacted Assistant Administrator (AA) Aurora Rigon who joined LPA during this visit.
During this visit, LPA reviewed current & past clients, & staffing with CG1 prior to arrival of AA. LPA reviewed same with AA & discussed allegation. Allegation of staff yelling @ clients denied.
The Department has investigated the above allegation & determined it to be Unsubstantiated.
