Licensing Program Analyst (LPA) D. Wright contacted Administrator Elizabeth Dyer via telephone due to COVID-19 and pre-cautionary measures.

LPA reviewed records and conducted interviews. LPA could not determine whether or not resident's needs are being met. Although the allegation might have happened and may be valid, there is not a preponderance of evidence to prove the allegation did or did not occur, therefore the allegation is unsubstantiated.

A copy of this report was provided to Elizabeth via email and an electronic email read receipt confirms receiving these documents.
