13 | On 06/09/2023, Licensing Program Analyst (LPA) Walton arrived unannounced to deliver findings on the above allegation. LPA introduced self, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Standards Compliance Coordinator, Cheryl Cardoza and Administrator, Scott Richards.
During today's visit LPA conducted staff interviews. Based on record review, the allegation: Staff left resident in feces for an extended period resulting in hospitalization is UNSUBSTANTIATED. Although the allegation may have happened or is valid there is not a preponderance of evidence to prove the alleged violation did or did not occur.
No deficiencies issued. Exit interview conducted and a copy of the report was discussed and provided to Administrator, Scott Richards, whose signature on this form confirms receipt of this document. |
