On 6/6/2022, Licensing Program Analyst (LPA) K. Kaur arrived unannounced at the above facility to conduct
an Annual Inspection- Infection Control. LPA introduced self, stated the purpose of the visit, and was informed that Administrator Bally Singh was not available.

LPA contacted Administrator via phone and was informed the facility closed two years ago, when COVID happened. Administrator stated they returned the Facility license and the residence is now an Air B and B. I asked administrator if they had informed anyone or discussed the final walk through. Administrator stated they were operating for twenty-six years and were not aware of any final walk through or inspection that was necessary.
