13 | On 11/07/23 Licensing Program Analyst (LPA) V Gorban visited the facility stated above to deliver findings. LPA met with Business Office Director Jennifer Fowler, explained the purpose of the visit. Administrator Patrick Frazier was notified of licensing visit. LPA toured the facility inside and out, observed residents in care and discussed findings to allegations.
Allegation: facility does not have an administrator.
During this investigation department reviewed facility file and interviewed administrator and staff. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. We have found that the complaint was unfounded, therefore we have dismissed the complaint.
