13 | On this date, Licensing Program Analyst(LPA) D. Ayers arrived at the facility unannounced to deliver complaint findings. LPA met with Beronica Galindo - Business Office Manager and announced the purpose of the visit.
During the course of the investigation, the Department conducted interviews, reviewed records, and conducted facility inspections. Beginning 10/1/2021, facility staffing had been increased and augmented by temporary staff from an outside agency. Based off of records, the facility had enough staff to meet the needs of the residents. The licensee had been actively attempting to hire permanent staff. A nurse consultant had been hired to provide support to staff. Residents with wounds are seen weekly by a physician and are routinely cared for by staff. Per medication records and interviews, no unqualified staff were administering medication. The above allegations are unsubstantiated. Exit interview conducted. A copy of the report was provided to the licensee via email.
