32 | On 3/12/26 Four staff (S1-S4) were interviewed. All staff interviewed stated that incident reports are written up and provided to parents when a child gets injured. Regarding the allegation that a lack of supervision resulting in a child injuring another child, S1 stated that there is one teacher and one aide that supervising a group of 4-5 children to ensure adequate supervision. Regarding supervision, S2 stated some days there are more staff than children. S3 stated that children are counted throughout the day, that staff never have their backs to the children and that staff position their bodies to be able to properly supervise children.
One child (C1) was interviewed on 3/12/26. C1 stated that no one bites or scratches me at school.
One witness (W1) was interviewed and stated that they would not be informed of what happened when their child came home with bites or scratches. W1 stated when they would ask staff for information on what happened, the staff would not have information on what happened.
On 3/12/25 at 10:00am Licensing Program Analyst Emily Curiel conducted an unnannouced visit at Villa Avenue Head Start. LPA Curiel observed two staff outside talking while children played out of their eyesight. Specifically, there was a child on a trike that was not in the line of sight and was off to the side.
Due to conflicting information obtained throughout the course of the investigation, the following allegation was determined to be unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred. Appeal rights were provided and exit interview conducted.
A Notice of Site Visit was provided and must remain posted for thirty days.
