25 | On 9/4/24 at 1:35 PM, Licensing Program Analyst (LPA) Gloria Gonzalez conducted an unannounced Case Management inspection to follow-up on a self-reported incident that occurred on or about 8/22/24. LPA disclosed the purpose of the inspection and was granted a tour into the facility by Helper, Martha Cecena. There were three (3) children and one (1) staff member present at the time of the inspection. Licensee arrived on or about 1:50 PM.
The incident that allegedly occurred on or about 8/22/24, where Licensee was notified on 8/23/24 by Parent 1 (P1) , was regarding Child #1, (C1) disclosed to P1 that someone at the daycare hit him on the arm and had a scratch or pinch
. Licensee stated that she nor her helper observed any incident where C1 was hurt nor saw a mark on his arm. Interviews were conducted with the Licensee, Helper, P1, and two daycare children. P1 declined the department to interview C1.
At this time, based on information obtained, no disclosures were made an no evidence to corroborate the allegation. A licensing violation has not occurred. No deficiencies cited. The incident investigation may be reopened should further information be obtained.
Licensee was provided a copy of the appeal rights (LIC 9058). LPA provided notice of site visit (LIC 9213) and observed it being posted at the facility. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
An exit interview was conducted with Licensee, Guadalupe Vizcarra. |
