25 | On July 31, 2026 at 11:31 AM, Licensing Program Analyst (LPA) Oscar Picazo conducted an unannounced Case Management inspection for the purpose of issuing a citation in conjunction with complaint investigation findings dated July 30, 2026. LPA met with Licensee, Ana Maria Palacios Torres. LPA disclosed the purpose of the inspection and was then granted access into the facility by the licensee. There were five (5) children and one (1) staff members present at the time of the inspection.
During the course of the complaint investigation, Licensee admitted that she placed Child #1 (C1) for a brief period in an infant rocker on the floor. This is a prohibited item for use per California Code of Regulations, Title 22, section 102417(g)(10). Upon further review of the rocker, it was determined that the United States Consumer Protection Safety Commission (CPSC) has issued an urgent official safety warning regarding this specific rocker. Licensee stated that she has removed the rocker from the day care as of 12/02/25.
Based on the interview and licensee's own admission statement, per California Code of Regulations, Title 22, Division 12, Chapter 3, one Type A deficiency is being cited on the attached LIC 809D.
LPA Oscar Picazo informed the Licensee Ana Maria Palacios Torres that this report dated July 31, 2026, documents one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
LPA informed the Licensee Ana Maria Palacios Torres to provide a copy of this licensing report dated 07/31/2026 that documents any Type A deficiency to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
