25 | On 05/24/2023, Licensing Program Analyst (LPA) Nancy Her conducted an unannounced case management inspection at the facility. LPA met with Site Supervisor Angie Esparza, to discuss an incident which occurred on 04/27/2023. A complete file review was conducted prior to visit. LPA toured facility inside and outside. LPA took a census and interviewed Site Supervisor.
On 04/27/2023 a child was left without the supervision of a teacher, including visual supervision which resulted in the injury of a child in care. Site Supervisor reported incident to Fresno Community Care Licensing on 05/04/2023.
Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiency is being cited.
LPA Nancy Her informed facility representative Angie Esparza that this report dated 05/24/2023 document one Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Nancy Her informed the facility representative to provide a copy of this licensing report dated 05/24/2023 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the facility representative Angie Esparza.
A notice of site visit was given and must remain posted for 30 days.
