On 10/10/2024, Licensing Program Analyst (LPA) Christopher Burnias and Licensing Program Manager (LPM) Luisa Gavoutian met with Acting Director Alicia Howard at the facility to conduct a case management inspection related to an Unusual Incident that was reported to the Department. The reported incident indicated that two children’s restrooms flooded and were under construction.

LPA and LPM toured the facility inside and outside and took a census. LPA and LPM observed that the children’s restrooms located in the 300 building are currently being renovated and are not currently in use. Acting Director informed LPA and LPM that the restrooms located in the 100 and 200 buildings as well as in the Worship Center located on the property, are temporarily being used by children in care. Acting Director stated that permission slips have been signed by parents to allow for field trips to the Worship Center. LPA and LPM inspected the restrooms located in the Worship Center and observed that there are 9 toilet stalls able to be used by children in care. LPA and LPM reminded Acting Director that complete supervision must be provided at all times even when taking children to use the restrooms at the Worship Center.

During the tour of the facility. LPA and LPM observed that classroom 307 and 314 have also sustained damage due to the flooding and are also being currently renovated. Facility Staff indicated that renovations for classrooms and restrooms are scheduled to be completed by Tuesday, October 15, 2024. LPA and LPM also observed that Classroom 306 is currently being used as a storage room. Facility staff indicated that they will have the room cleared or submit an application to reduce capacity size. Staff will notify the Department of their decision by 10/18/24. In addition, it was observed that the name of Classroom 311 has been changed to Classroom 312.

Upon review of the Unusual Incident Report submitted by the facility Director, LPA and LPM determined that the facility did not submit an Unusual Incident Report within the required timeframe. LPA and LPM discovered that the facility submitted an Unusual Incident Report to the Department on 09/30/24 to report an incident that occurred on 08/16/24 where the toilets in the children’s restrooms had flooded. Departmental records show that the facility called the Department on 09/30/24 to report the same incident that occurred on 08/16/24. LPA and LPM determined that reporting requirements were not met as the facility did not report the incident within the Department’s next working day by phone requirement or within seven days in writing requirement. A citation has been issued (See attached LIC 809-D).

Upon staff record review, LPA and LPM determined that although the facility has reported Alicia Howard as Facility Director as of March 2024, Ms. Howard does not meet education requirements to be a facility director. A review of Ms. Howard’s records showed that they have not completed required three units in each of the following categories: Administration, or Staff Relations, Child, Family and Community, and Care of Infants. Ms.Howard is currently enrolled in courses for Infant Development and Administration, and is expected to complete the courses in December 2024.

During the review of staff records, it was determined that Staff 1 (S1) met requirements to be a Facility Director. S1 will submit a Director Packet Application to the Department within 10 days.

Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, the following deficiency is cited (See attached form). Exit interview conducted and report was reviewed with Acting Director Alicia Howard. Acting Director was provided appeal rights. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.

Based on document review, the facility submitted Unusual Incident Report and called the Department beyond required timeframe. Facility notified the Department 44 days after the incident occurred. This poses a potential health, safety, or personal rights risk to children in care.
