On September 28, 2023 Licensing Program Analyst (LPA) Kari McWilliams conducted an unannounced case management visit and met with Director, Wendy Mayes. LPA McWilliams toured the facility inside and out and a census was taken.

LPA McWilliams informed Director Mayes the purpose of the visit was due to the facility being on a required list following a noncompliance conference on February 25, 2019 in response to care and supervision incidents that were reported to the Department in 2017 and 2019.

LPA McWilliams toured the facility, interviewed Director Wendy Mayes regarding the supervision training's that staff are completing annually at the staff in-service in August, as well as the new staff are taking the supervision training on the website, www.cdss.ca.gov. LPA confirmed that all staff are adhering to supervision expectations that were in alignment with Licensee's policy and procedures and the outlined expectations that were in the noncompliance conference summary.

During todays inspection LPA McWilliams did not observe any concerns with care and supervision.

Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited.

Exit interview conducted and report was reviewed with the Director Wendy Mayes.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
