On September 6, 2024, an Informal Office Meeting was conducted at the Fresno South Regional Child Care Office. In attendance at the meeting was Director, Sarah Vinson and Administrator Lynda Vinson. Also present was Licensing Program Manager, (LPM) Luisa Gavoutian and Licensing Program Analysts, (LPAs) Paul Garcia and Octavia Nolan.

The purpose of this meeting was to discuss recent violations of Title 22 regulations, that if not corrected, would pose an immediate and potential risk to the health, safety, and personal rights of children in care.

The following issue/violations was discussed:

Type A Deficiency cited on August 13, 2024
101233(a)(3) Personal Rights.

Based on interviews conducted and records obtained during the Complaint Report investigation completed by LPA Paul Garcia with the Department of Social Services Community Care Licensing Division, it was discovered that Director Sara Vinson permitted child #1 (C1) to consistently hurt many of the community’s children in the preschool over several years that resulted in insufficient protection to children related to enhanced child-on-child aggression.

Based on interviews conducted and records obtained it was discovered Canyon Hills Preschool failed to report numerous Unusual Incident Reports to the Fresno Community Care Licensing Division as required per a condition of their license. Continued on LIC809-C

Based on interviews and documents obtained the licensee failed to follow their own Discontinuation of Services Policy in their Plan of Operation pertaining to protecting children or persons at the facility.

Personnel Requirements
On August 13, 2024, a Case Management report documented that Canyon Hills Preschool Shall be required to provide an additional staff member that shall not be included in the center's staffing plan to solely supervise child #1 (C1) at all times and Canyon Hills Preschool shall also provide an additional staff member to solely supervise any child exhibiting reasonable similarities of child-on-child aggression with similar frequencies remotely resembling C1s documented conduct.

Corrections for all deficiencies were received at the Fresno Regional Office before August 29, 2024.

Per Directors request, the facility has been referred to the Technical Support Program (TSP) for assistance.

It was discussed that continued violations of Title 22 Regulations and failure to maintain compliance will result in a Non-Compliance conference and may be referred to our Legal Division for possible Administrative Action.
