Licensing Program Analyst (LPA) conducted an unannounced Case Management visit in regards to an alleged physical abuse reported by the facility. LPA Williams met with Administrator Diedra Grigsby, and discussed the purpose of the visit.

LPA Williams toured the facility with the Administrator.

LPA Williams interviewed Resident 1.

LPA Williams received facility and resident documents from the Administrator.

The Administrator placed Staff 1 on leave until further notice.

Due to insufficient information at this time, further investigation is required.

An exit interview was conducted and a copy of this report was provided.
