32 | Pest control records indicated that the initial service addressing the concern was conducted on 04/04/2026, 17 days after the notification. However, according to pest control service documentation and interviews, pest technicians did not observe any active pest activity during the 04/04/2026 service or the follow up service on 04/18/2026.
Based on the information gathered through observations, interviews, and record review, the investigation revealed that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur. Therefore, the allegation is UNSUBSTANTIATED.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency is cited today.
An exit interview was conducted, and the report was reviewed with Senior Director Gladys Garcia-Jara. This report shall be made available to the public upon request. The LIC 9213 Notice of Site Visit was provided and is required to be posted for 30 days. Appeal Rights were provided to the Licensee. |
