On 4/10/2024, Licensing Program Analyst (LPA) Theresa Marquez conducted a Case Management inspection and met with Director Cheri Mosley. The purpose of this inspection is in regards to a self reported incident telephoned to the Officer of the Day by Director Mosley on 4/9/2024.

On 04/09/2024 at 9:03 AM, Staff 1 returned to the Toddler Classroom and discovered Child A was left alone inside the classroom. Staff 1 immediately notified Director Mosley and Staff 2.

Staff 1 and Staff 3 had taken 14 children outside for the AM recess. Staff 2 remained in the classroom supervising 5 children while they were taking their school pictures.
Staff 2 and 4 of the children left the classroom, but Staff 2 was not aware that Child A was still inside the classroom. Staff estimated that Child A was left alone in the classroom for about 3 minutes. The parent of Child A was immediately notified of the incident and Staff 2 was immediately terminated.

Per Director Mosley, the facility has a counting system, when the teachers and children enter or exit the classroom from the playground, the teachers are required to take census and record the census on the clipboard to ensure that they do not leave any children unsupervised. Staff 2 failed to keep an accurate count of the children in her care and provide adequate supervision.

The LIC624 Unusual Incident report was provided to LPA Marquez today and US mailed to the licensing office on 4/9/2024..

LPA Marquez determined that although adequate ratios were in place when the incident occurred and Director complied with timely reporting requirements, a lack of proper supervision resulted in the child being left unattended.

Per California Code of Regulations Title 22 Division 12 Chapter 1, the following deficiency is being cited today (See page LIC9099-D). A child was left unsupervised in a classroom.

LPA Marquez informed Director Mosley that this report dated 4/10/2024 documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

LPA Marquez informed Director Mosley to provide a copy of this licensing report dated 4/10/2024 that documents a Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

An exit interview was conducted and report was reviewed with Director Cheri Mosley. A copy of Appeal Rights and the Notice of Site Visit was given and must remain posted for 30 days.
