On 08/22/2024, Licensing Program Analyst (LPA) Lady Cabrera conducted an unannounced complaint inspection at the facility. The purpose of the inspection was to deliver the finding for the above listed complaint allegations.

During the course of the investigation, LPA Cabrera collected facility records and conducted interviews of facility representatives, staff and parents. LPA was unable to obtain additional information corroborating the level of injury the child sustained while in care. Per records reviewed, child injured his/her hand with the outdoor play area chain link fence. Staff cleaned the injury area and provided first aid. Parents were notified via incident report form.

On 07/01/2024, LPA observed the outdoor play area chain link fence with the Director. LPA observed some exposed metal pointed edges that could potentially be a hazard. Director submitted a work order to address the exposed metal edges.

 On 08/22/2024, LPA tour the outdoor play area with the Director to check the chain link fence. LPA observed the chain link fence was repaired.

The investigation revealed through interviews and review of records, that although the above allegatiosn may have happened or is valid, there is not a preponderance of evidence at this time to prove the alleged violation did or did not occur, therefore the allegations are UNSUBSTANTIATED.

Per California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency is cited during today's visit. An exit interview conducted with Director Mary Ann Moreno. A copy of this report and Appeal Rights were provided and discussed with Director Mary Ann Moreno.

A Notice of Site Visit Form was posted to parent's board and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
