25 | On May 21, 2024 Licensing Program Analysts (LPAs), Kari McWilliams and Chris Burnias conducted an unannounced case management inspection. LPAs toured the facility and a census was taken. LPAs met with Director Mary Ann Moreno and informed Director Moreno the purpose of todays inspection.
On April 25, 2024 it was reported the to Fresno South Regional office that on April 23, 2024 child #1(C1) was running in the wood chips chasing another child and tripped over the edge of the wood chips box area, when they fell and landed on their hands. C1 was observed holding their wrist when they got back up. Staff #1 (S1) was standing about 10 ft. away from child and observed C1 trip and fall. C1 was taken to the office and an ice pack was applied. Parent/Guardian was notified and picked up C1. It was reported that C1 was taken to urgent care and diagnosed with a hairline fracture on her elbow.
During today’s inspection LPAs observed the area of the wood chips, interviewed staff involved and gathered more information. LPA observed that there was a 10 inch difference between the bottom of the wood chip box and the top making it a tripping hazard to children in care.
It was reported that there were 31 children present outside when the incident occurred and there were four staff present outside.
C1 returned to the facility on April 29, 2024. Director Moreno reports that child attends Monday, Tuesday and Thursdays. C1 returned with no restrictions, however guardian requested C1 to stay out of the wood chip area and the sand area.
