On 10/04/22, Licensing Program Analyst (LPA), Jose Penate met with Director, Ashley Herrera for an unannounced case management inspection. LPA toured the facility and a census was taken. The purpose of today’s inspection was to follow up with Ashley regarding a the director change. Upon discussion with Ashley and LPA observance while conducting inspection, it was determined that the center has been and is currently without a Qualified Director.

LPA Penate has been in contact with Administration via email and telephone for paperwork and qualifications for new director, unfortunately documents as of now still have not been submitted to Community Care Licensing (CCL) to be verified that new appointed director has full qualifications. LPA reviewed documents with Ashley, and it was confirmed that documents have not been completed and submitted to CCL to verify newly appointed director qualifications.

Per Chapter 1, Division 12, Title 22 of the California Code of Regulations, deficiency cited during today’s inspection (See LIC809-D).

Exit interview conducted with Ashley. This report is to be made available to the public upon request.

LIC 9213 Notice of Site Visit to be posted for 30 days.
