25 | On 06/19/24, Licensing Program Analyst (LPA) Denisia Jimenez arrived at the facility to conduct an unannounced Case Management inspection. LPA met with Director, Ashley Herrera. LPA toured the facility, and a census was taken. The purpose of today's inspection was regarding an unusual incident that was reported to the Fresno Childcare Regional Office on 06/04/24. On 06/03/24 Child #1 was left unattended by Staff #1 in the preschool classroom for approximately 2 ½ minutes. Parent #1 observed Child #1 alone and contacted Staff #2 who took Child #1 to his classroom. LPA and Director discussed facility policies and procedures. Director stated she will be conducting training with all her staff on supervising the children. Though the Director reported the incident in which the child was alone in the preschool classroom and has initiated corrective plans moving forward in ensuring children are always accounted for, child number one was at immediate risk when an un-fingerprinted adult found the child alone and alerted the site staff. As a result, a type A deficiency is issued along with a $500.00 immediate civil penalty assessment.
Exit interview conducted and report was reviewed with Director, Ashley Herrera. Appeal rights were provided.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, the following deficiency is being cited: (see next page).
Upon receipt of a Type A violation, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. A copy of the Fact Sheet - Child Care Parent Notification Requirements and a copy of LIC 9224 Acknowledgement of Receipt of Licensing Reports was given to Licensee.
This report shall be made available to the public upon request.
