13 | On 09/04/24, Licensing Program Analyst (LPA) Denisia Jimenez conducted an unannounced complaint inspection at the facility to deliver finding for the above-mentioned allegation. LPA met with Assistant Director, Haley Harl. LPA explained and discussed the above allegation, and a census was taken.
During the investigation, LPA interviewed Complainant, Director, Assistant Director, and Day Care Staff. LPA also reviewed facility records, requested a copy of the children’s roster, obtained documents, and a personnel report.
Interviews revealed that although parent #2 of Child #1 stated that Child #1 had a bruise on its eye during pick up time, staff stated that they did not see a bruise on Child’s eye when parent #2 brought it to staff’s attention. Staff stated when a child gets injured at the facility an incident report is provided to the parent. Staff also stated if a parent brings up an unexplained injury, they will ask the other staff that was in the classroom or send the parent to speak with the director. LPA was unable to verify if day care child sustained an unexplained injury while in care.
