25 | On 04/23/2026, Licensing Program Analyst (LPA) Denisia Jimenez arrived at the facility to conduct an unannounced Case Management inspection. LPA met with Site Supervisor, Yesenia Pineda Duenas. LPA toured the classroom, and a census was taken. The purpose of today’s inspection was to follow up on an unusual incident reported to the Fresno Child Care Regional Office on 04/15/26. It was reported that on 04/15/26, Child #1 was running on the carpet in Classroom 9, tripped, and fell forward, striking their head on a cubby. The child sustained a cut above their right eyebrow. Staff applied pressure and a cold compress, and the child later received stitches. Child #1 is currently not attending the center. Staff and the Site Supervisor were interviewed, and additional information was obtained.
Based on the information obtained today, it was determined that the Site Supervisor responded to the situation promptly and appropriately, and the incident was reported within the required time frame in accordance with reporting requirements. Through interviews, records review, and observations, it was noted that the facility adheres to established supervision protocols by ensuring children are consistently supervised, and the facility demonstrated transparency and compliance with reporting requirements. At this time, no definitive information was obtained to substantiate the incident occurred; therefore, further investigation is required.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited during today's visit.
Exit interview conducted with Site Supervisor Yesenia Pineda Duenas and a copy of the report and Appeal Rights were given and discussed.
