On June 30, 2022 Licensing Program Analyst (LPA) Kari McWilliams conducted an unannounced inspection to clear a plan of correction. LPA McWilliams met with Director Amber Lytvynenko. A tour of the facility was conducted inside and outside and a census was taken.

LPA McWilliams went through the medication and observed all medication was not expired and was properly labeled with appropriate plans with the medication and plans in the child's file. All medication was locked and out of the reach of children.

Exit interview conducted and report was reviewed with the facility representative, Amber Lytvynenko .

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited.

This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
