25 | On April 8, 2026, Licensing Program Analyst (LPA) Xona Xayavong conducted an unannounced case management – incident inspection. LPA met with Director Rosita Barron. LPA toured the inside and outside of the facility, and a census was taken. The purpose of today’s inspection was to follow up on an unusual incident report received by the Fresno Regional Office on February 26, 2026.
On February 26, 2026, at 10:30 a.m., on the Sea Otter’s playground, Staff #1 (S1) observed Child #1 (C1) jump off the wooden stepping blocks onto the artificial grass. S1 went to C1 and asked if they were okay and if they could get up on their own. C1 stood up and walked over to the bench area where Staff #2 (S2) was located. S2 assessed C1 and observed swelling on C1’s right ankle. Staff applied an ice pack to C1’s right ankle. C1’s mother was contacted, and C1 was taken to the doctor on February 27, 2026, where a cast was placed on C1’s right leg. C1 has returned back to the facility on March 23, 2026.
During today’s visit, LPA conducted staff interviews and observations. Based on interviews, LPA determined that staff took appropriate actions and there was no lack of supervision. Based on observations, LPA did not observe any hazards accessible to children in the area where the incident occurred that could have contributed to C1’s injury.
An exit interview was conducted, and the report was reviewed with Director Rosita Barron. Appeal rights were provided. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiency was cited. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit was provided and is required to be posted for 30 days.
