32 | Although there was sufficient staffing for the ratios, the Director was provided the policy for an Infant Teacher, Infant Teacher’s Aide, Teacher, and Teacher’s Aide as the Director stated during interview she was unaware of all of the course requirements needed to have those positions. Technical Assistance was provided for the policy requirements for Teachers and Aides.
At 11:54am, LPA observed Staff 2 providing care and supervision to children in care in addition to one qualified teacher and two teacher aides. Staff 2 was acting as an infant teacher aide in the infant classroom but stated that she acts as a floater for the preschool license, as well. Staff 2 stated during interview that she does not have any child development or early childhood education courses taken or transcripts for the courses and is not currently enrolled in any institutions to acquire the courses needed to meet the Title 22 Requirements. Staff 2 further stated she intends next Friday to be her last day. The Director was made aware of the admissions made by Staff 1, the Director stated that she will not be providing assistance as an infant teacher aide. The Director stated she will only drive and cook until she leaves.
LPA observed Staff 4 providing care and supervision in the preschool classroom; however, there two children who were left unattended by a book case out of sight of the Teacher without supervision and there were two children witnessed being in the bathroom with the door open by themselves without supervision. This is a Type B violation, see the LIC 809D for details.
LPA reviewed the file of Child 1 and seen that the sign-in sheet had not been completed by her parents during pick-up and drop-off. The Director stated that parents don’t always listen and sign-in and out when they are supposed to and the teacher may miss it while attending to other children. This a Type B Violation, see the LIC 809D for the details.
As a Type A deficiency has been cited and a civil penalty assessed of $100/day for a total of $500, a copy of the citation and licensing report must be posted for 30 days. The same report must be provided to Parents/Guardians and the Acknowledgment of Receipt of Licensing Reports LIC 9224 must be signed by Parents/Guardians of all enrolled children and any newly enrolled children in the next 12 months following the citation. If these requirements are not met, civil penalties per violation will be assessed.
An exit interview was conducted, a copy of this report was read and provided to the Director Melody Sweany, and the Notice of Site Visit and Appeal Rights was explained and given. |
