25 | On 02/09/2023, Licensing Program Analyst (LPA) Theresa Marquez conducted an unannounced Case Management inspection and met with Site Supervisor, Kina Amos Gandy. The purpose of today's inspection is to address the lead exceedance referred to as an Action Level Exceedance (ALE) of over 5.5 parts per billion (PPB) in the water at the facility.
The Fresno South Regional Office received notification of water lead exceedance at the facility resulting from a recent water lead sampling test conducted on 12/10/2022. Based on records reviewed from Child Care Center Lead Sampling and Reporting Tool, outlet named B was impacted with an (ALE) of 23.6 PPB and outlet named C was impacted with an (ALE) of 81.8 PPB. Outlet B, a tilt skillet faucet and outlet C, a 'pot filler' spray hose are both located in the elementary school kitchen. These results indicates out of compliance of over (ALE) 5.5 PPB. (See Plan of correction for further corrective action see page LIC 809-D). Outlets B and C have been temporarily made inoperable.
Inside the classroom there is 1 water fountain which had a "none detected" lead level and is used for drinking purposes. The outside play yard has 2 water fountains which also had a "none detected" lead levels.
During this inspection, LPA Marquez spoke to Director Kim Reedy and advised her of this inspection and citation. LPA Marquez then met with Josh Rogers, Director of Nutrition Services. Rogers and LPA Marquez observed and discussed outlets B and C which have been taped off for non use by staff. Rogers agrees to continue with the re-sampling process for an action level result of under 5.5 PPB for compliance with a Certified Sampler.
Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, a type B deficiency is being cited: (see next page, LIC 809-D). An exit interview was conducted with Kina Amos Gandy and this report was reviewed and provided. A Notice of Site Visit (LIC 9213) and copy of Appeal Rights was also provided. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days. |
